Part time LPNs (LVNs) and Social Workers needed - Immediately

Camelot Care LLC is currently searching for social workers, LPNs/LVNs, RNs, with a clean background, and at least one year of nursing experience in Skilled Nursing, Long Term Care, and/or Home Health, to act as Patient Care Managers in Lexington/Winston Salem, NC.
The program involves weekly in-home visits to patients that have recently been released from the hospital. Nurses would make weekly in-home visits with these patients and develop a care plan for them to avoid re-hospitalization. This position involves nothing clinical or hands-on, it consists mainly of patient education along with online documentation of the patient's situation.
This position offers complete flexibility, as you can write your own schedule. Nurses are free to take responsibility for as few or as many patients as desired, so long as the patients can be seen on a weekly basis. We have nurses who participate in the program, start as part time and later maked this their full-time income, as well as nurses who just do it on a part-time schedule to pick up some extra money.
This opportunity offers competitive compensation based on the number of visits you complete.
Must have had your LVN/LPN or Social Worker license for at least a year!
Estimated Salary: $20 to $28 per hour based on qualifications.
